Section 1:
Requesting Individual is - Check the box indicating the person making the request to participate in the project.

Section 2:
Information on Participating Veteran - Fill in the name, address, etc. of the VETERAN, no matter who is submitting the request.

Section 3:
DD Form 214 Attached - Check the box indicating that the DD Form 214 is attached. Forms without a DD Form 214 will be returned.

Section 4:
Participate in (levels) - Check the boxes that apply to the type(s) of participation.

Section 5:
Payment Enclosed - Check the box indicating form of payment. PLEASE DO NOT SEND CASH. Please send money order, personal check, MasterCard/Visa charge card and fill in the amount on the line. If using charge card, please fill in ALL of the areas in the MC/VISA section.

Section 6:
Current Veteran Status - Please check the appropriate box. If the veteran is deceased, please check off the appropriate boxes related to his/her death and fill in the date he/she passed away. To prevent confusion in the date, please use the 3 letters of the month, i.e., JAN 04 1967.

Section 7:
Branch(es) of Service (Active Duty & Reserve) - entry date, discharge date and rank attained. Please use the following terminology/codes - Branch of Service: USA, USAF, USCG, USMC, USN (Active Duty) - RES-USA, RES-USAF, RES-USCG, RES-USMC, RES-USMC (Reserves) - NG (National Guard), ANG (Air National Guard)..Beginning and Ending Dates - These are the dates of entry and discharge - Use the 3 letters of the month so there is no question on the date, i.e. JAN 04 1967 - Rank/Grade - Use grade, rate or rank abbreviations such as COL, SSGT, SP4, LCPL, MGSGT, followed by the Officer or Enlisted pay grade, such as O3, E5, etc.

Section 8:
This section is the heart of the locator service. In a “typical” period of service, 4 units and/or locations should be enough to describe when and where you served. Division/Group/Theatre/Wing/Battle Group - use the terminology that most people will know, such as Americal, 25th Infantry Div, 9th Air Force, I Corps, Pacific Fleet, etc. Unit/Ship/Platoon/Squadron/Company - use common terminology that will be recognizable to those looking for you, such as the name of the ship(s) you served on, the platoons, units, squadrons, etc. Camp/Location/St./County - use the names of the Camp, Base, Fort, Post, LZ, Ocean, Sea, Town, Province and the state or country they are in. Arrived & Departed Dates - please use the 3 letters of the month, i.e. JAN 04 1967.

Section 9:
This section is only for use if the requesting individual is NOT the veteran whose information is being supplied. This information will be used to contact the requesting individual if further clarification or information is needed.

Photograph:
If a photograph is included, IT CANNOT BE RETURNED, so please send a copy. It will be reduced (or enlarged) to 4” x 4” when it is scanned into the computer. Please provide a caption for the photo that identifies who is in the photo, using references such as, (L-R) Sgt. John Doe, SP-4 Mike Smith. Please also include the date and location if known.

Biography:
A 1200 word biography will fill two 8” x 11” sheets of paper if it is typed, single spaced. The biography should be descriptive of the veteran‘s activities and accomplishments. Include high school attended, year of graduation, sports involvement, schools attended after high school, military academy attendance, where they were stationed for basic or OCS training, where they served during their period(s) of service, what they did in the service, what they did when they returned home, who they married, the names of their children and/or grandchildren, what they are doing today, any involvement in community service projects, and any veteran‘s organization
